There are several methods for creating nodes in NVivo - we prefer the option of right-clicking in List View and then choosing 'New <node type>' from the pop-up menu that appears. (Note that <node type> will be replaced with the name of the node you are trying to create.)

When creating new nodes in NVivo, don't forget to add a description into the field provided - this is an extremely useful way to keep track of your thinking around your categories and how they're developing.
